Requirements for a Florida Driver License or ID card

  • Original Operator Driver License - Transfer From Out of State
  • Two forms of identification, a primary and secondary, as described in information that follows. The names MUST match exactly. i.e. Your out of state driver license and social security card.
  • Vision test, with or without corrective lenses, to meet acuity of 20/40.
  • Hearing test.

Original Operator Driver License - First Time License - Age 15 or Older

  • 2 forms of identification, a primary and secondary, as described in information that follows. The names MUST match exactly.
  • Traffic Law and Substance Abuse Education Course (TLSAE). This course is provided by outside companies, and can be located by looking in the phone book yellow pages, under Driver Instruction. Driver Education Students may have a form, that will meet this requirement, from their school.
  • Under age 18 will need a parent with them to sign a Parental Consent Form. This can only be done by a Natural Parent. If there is a Guardianship, bring a Certified Copy of Court Documents, showing there is Court Appointed Guardian.
  • A written test is required, which includes 20 questions of road rules, and 20 questions of road signs. The test may be waived for Driver Education Students, with proper documentation from their school.

Non-Resident Driver License - Original Valid in Florida Only Driver License

  • 2 forms of identification, out of state driver license and one other document, such as a social security card. The names MUST match exactly.
  • Vision test, with or without corrective lenses, to meet acuity of 20/40.
  • Hearing Test.

NOTE: This non-resident driver license is for use only in the State of Florida. When you travel outside of Florida, you will be operating on your Out of State License. You will not be considered a resident of Florida, and may not be eligible for some rights and privileges of Florida Residents.

Original Commercial Driver License (CDL) - Transfer From Out of State

  • Two forms of identification, a primary and secondary, as described on the list that follows. The names MUST match exactly. Usually this is your out of state CDL and Social Security Card. Commercial Driver License Operators are required to present a Social Security Card for issuance of a Florida CDL.
  • Vision test, with or without corrective lenses to meet acuity of 20/40.
  • Hearing test.
  • Hazardous Materials Test, is required if it has been more than 2 years since the issuance of your license. All other tests and endorsements are reciprocated.

Original Identification Card - All First Time Issuances of a Florida ID Card.

  • Two forms of identification, a primary and secondary, as described on the list that follows. The names MUST match exactly. ID Cards can be issued as "Resident-Yes" or "Resident-No". A "Resident-Yes" ID card will only be issued if you meet, and can prove one of the following requirements:
    • Surrender your out of state license.
    • Children enrolled in the Florida School System.
    • File a Declaration of Domicile with the Florida Court System.
    • Proof of Employment in Florida. (A letter must be on employers letterhead.

NOTE: Florida does not have reciprocity with some states and you may be required to show a primary document of identification.
